General Statement of Duties The Home-Based Treatment Worker provides 1:1 intervention to children with intellectual disabilities and mental health care needs in a home and/or community setting
Job Responsibilities
- Provide treatment in the home and/or community setting
- Implementation of child’s individualized Treatment Plan
- Collect data on responses to interventions for each treatment goal
- Be knowledgeable of the child’s primary diagnosis and treatments
- Attend all scheduled supervisory sessions and required in-service trainings
- Complete and submit Incident Reports to the Children Service Coordinator
- Always maintain confidentiality to ensure the child and families rights to privacy
- Be knowledgeable and adhere to regulations set forth by the Department of Children, Youth and Families (DCYF) when suspecting child abuse and/or neglect
- Report as assigned to the Clinical Supervisor
- Adhere to all Fogarty Center Policies and Procedures
Qualifications
- Associate Degree in the human service field or High School diploma or equivalent.
- Two (2) years of supervised experience working with children with special health care needs
- Friendly, approachable attitude and willingness to assist others
- Commitment to family centered care
- Accuracy and attention to detail
- Strong communication skills, both verbal and written
- Team-player who appreciates collaboration
- Eager to play!
