Home-Based Treatment Worker

General Statement of Duties The Home-Based Treatment Worker provides 1:1 intervention to children with intellectual disabilities and mental health care needs in a home and/or community setting

Job Responsibilities

  • Provide treatment in the home and/or community setting
  • Implementation of child’s individualized Treatment Plan
  • Collect data on responses to interventions for each treatment goal
  • Be knowledgeable of the child’s primary diagnosis and treatments
  • Attend all scheduled supervisory sessions and required in-service trainings
  • Complete and submit Incident Reports to the Children Service Coordinator
  • Always maintain confidentiality to ensure the child and families rights to privacy
  • Be knowledgeable and adhere to regulations set forth by the Department of Children, Youth and Families (DCYF) when suspecting child abuse and/or neglect
  • Report as assigned to the Clinical Supervisor
  • Adhere to all Fogarty Center Policies and Procedures

Qualifications

  • Associate Degree in the human service field or High School diploma or equivalent.
  • Two (2) years of supervised experience working with children with special health care needs
  • Friendly, approachable attitude and willingness to assist others
  • Commitment to family centered care
  • Accuracy and attention to detail
  • Strong communication skills, both verbal and written
  • Team-player who appreciates collaboration
  • Eager to play!
